Cancel Urban Active Fitness

Urban Active Fitness clubs was a chain of fitness facilities based in Ohio, Kentucky, Tennessee, Georgia, Nebraska, North Carolina, and Pennsylvania that was acquired by LA Fitness in October 2012. To find out more, click here.

If you began with an Urban Active Fitness membership (now L.A. Fitness) and want to cancel your membership, proceed with the same steps as you would to cancel your L.A. Fitness membership.

As a reminder, here are the steps.

What You’ll Need:

Here are some possible items you will need to complete your cancellation:

  • Member number
  • Address of your local gym
  • Email on file
  • Reason for cancellation
  • Address on file
  • Your date of birth
  • Last four digits of your Social Security Number
  • Last four digits of your driver’s license
  • Username & password (if registered online)
  • This form: https://www.lafitness.com/Pages/PDFCancellationForm.aspx

 Steps to Cancel:

  1. Access the online cancellation form. Click on My LA Fitness, then login, click on “Account Information” tab.
  2. Then, select the “Cancellation Form” option on the right side of the screen.
  3. Complete the form. Keep in mind that missing fields will hold up the cancellation process, so make sure you review the “What you Need” section of the article.
  4. Mail the form to the address listed on the form or turn it into your local L.A. Fitness Operations Manager. Do this between 9 a.m. and 5 p.m. on Monday through Friday.
  5. Save a copy of the completed form for your records.
  6. Call the customer service phone number to check the status of your cancellation.
  7. Check your recent bank account or credit card statement to ensure that the membership cancellation went through.

Expert Tip:

  • Going in person to your gym location is usually accepted as a method of cancellation - just make sure you get it in writing
  • L.A. Fitness allows you to freeze your membership for a monthly fee of $10. If you are traveling or not planning on using the gym for an extended period, this is a great option. I reuqested to cancel my personal training sessions on March 14,2012 with Urban Active employee Jack, at their location. Since I have done so, I never got the refund I was promised for paying the first and last months dues for training sessions when I originally signed up. I went back in April as I was in Ohio to find out what was going on, and spoke to Andrew, who then let me know my request was denied. My bank statement Jack and I submitted online was not accepted as proof of moving to New York, since it was a screen shot. I asked if they were going to let me know- he didnt have an answer but said he would stop this months charge from going thru as I resubmitted another form as proof of moving to New York. Since then, I was charged another montly fee, so I contacted my bank to dispute the charge I was promised not to have. I also contacted Urban Active Corporate offices, explaining the situation, requesting my refund and that they cancel my membership, per contract. They told me since my account is now delinquent (since I disputed last months charge) they will not cancel my membership, and will not refund my money. The contract states if I move out of state or somewhere there is no Urban Active within 50 miles, then I am able to get out of the contract with full refund and cancel the autopay. Urban Active does not have my authorization to continue charges which I let them know, and they told me they will continue running my card, until they accept proof that I have moved. I have submitted 2 pieces of information now- my bank statement and my health insurance mail- both of which they have denied as proof. I am now filing a consumer complaint. I do not believe this is legal, ethical, or the right way to handle. No complany can "deny" the fact that I have moved to another state, with the threat to continue charges without my authorization, or refund the money I was contracted to legally get back.

I want to cancell my membership, but it says i can only cancel if i move or have a medical reason, i just want to end this membership. What if i don't pay my membership fee of 19.95 every month and i no longer use the card.

Sorry for your frustration. If you're still receiving collection calls after you canceled your account, I would definitely contact Urban Active directly.

Urban active is the worst! I was told I was getting a 2 month membership for a personal trainer and signed a little kios machine for what I thought was 2 months of payments to my credit card. I never used the service and thought I was out of 120.00, my bad for not using it. They continued to withdrawl from my account each month and I contacted them to find out why.. after lying to me for months, telling me their corportate office was cancelling my membership ..etc.. the withdrawls continued. I was later told that I had signed a year contract but i was never given any contract paperwork and felt totally lied to. The more I tried to get to the bottom of this issue, the more run around I got. Finally I went into the gym and they had me sign again a cancellation.. I continued to get collection calls however and was told they had denied the cancellation and until I paid the yrs worth of dues, could not cancel me. After several attempts to try and work with someone else..etc. I got so frustrated I changed bank cards.. I was recently contacted again for payment and asked how much I owed.. they told me that my "contract" was set up to auto-renew and now they were charging me for another year!! Totally frustrated and unsure where to turn now.. Will try contacting the BBB as someone mentioned in previous post!

I and my sister signed up for personal training for 149 a month, 30 mins a week for 12 months. After 3 months my sister stopped going with me, and since it was on my account, I kept paying and going alone. However after 3 months, it was too much because my husband lost his job. I paid the first and last month up front, so 300, and I asked how to cancel on march 31. They said I had until April 12 to cancel, and use my 18 sessions I already paid for and hadn't used yet. On April 12 I would have to pay 274 dollars and forfeit any of the 18 sessions I didn't use yet. Since I already paid the last month, I feel like I shouldn't have to pay so much to cancel. Also if I paid for the sessions, why do I have to forfeit them and pay all that money? I said I couldn't afford to keep paying, so how am I supposed to come up with 274 dollars to cancel? I need help because I don't think this is fair at all.
